What this website does not collect

This site does not accept patient names, dates of birth, scans, radiographs, clinical photographs, or other protected health information. Clinical data is exchanged only through a submission method the laboratory confirms with your practice.

Form warning

Do not include patient names, clinical files, or sensitive health information in this form. Use the approved secure case-submission method.

Analytics and logging

Clinical information is never placed into analytics, application logs, public storage or ordinary email.
